Passengers on board told 9News the plane had climbed to around 600 feet when they felt the aircraft lose power.
It is understood the pilot veered left over Lake Connewarre and made a forced landing metres from the water.
A full-scale emergency response was triggered with tough terrain and 17 patients proving particularly challenging.
"Really great to see them work together to overcome this particular incident," Terry Marshall from Ambulance Victoria said.
"The paddock was growing crops that were a couple of feet high so it was quite difficult to actually get people from the location to helicopters and back of ambulances."
One man was airlifted to Melbourne with upper body injuries.

Paramedics took six others for treatment in Geelong while other patients also turned up for treatment of their own accord.
"When we hear of a plane crash, we always think the worst, particularly with 17 patients on board, so I think this time we've been extremely lucky," Marshall said.
Skydive Australia said its pilot training and knowledge allowed him to follow procedure and execute the landing as safely as possible.
The company said it will now work with authorities as a full investigation is carried out.
